David C. Welch DO, FACOEP |
Dr. Welch is the founder and owner of Ultimate Urgent Care Centers, LLC and comes to the urgent care medicine specialty with a long history of ambulatory medical experience. Dr. Welch was a co-founder and president of Phoenix Emergency Group, Inc. since it's inception in 1989. As such, he has served as the base hospital medical director at both rural and urban Emergency Centers. He has been devoted to EMS and has served as the EMS Medical Director for several fire departments and EMS agencies, and college paramedic programs. He is an active ACLS instructor and medical director for ACLS programs as well as currently teaching a monthly X-ray interpretation class for flight medics and flight nurses as part of Wizard Education's monthly Cadaver Lab. He was also one of the original Basic Trauma Life Support (BTLS) instructors for the American College of Emergency Physicians in Arizona. Dr Welch's commitment to medical education is exemplified by the fact he was the founder and past Program Director of the first Osteopathic Emergency Medicine Residency training program in Arizona. Currently, Ultimate Urgent Care Centers continues the philosophy of giving back to the medical community by hosting two medical students per month from Midwestern University's Arizona College of Osteopathic Medicine as a part of their clinical educational externships. Dr. Welch has also developed computer based emergency medicine quality assurance and continuing quality improvement programs and he has developed a platform independent Electronic Medical Record for use in urgent care centers and emergency departments.
Dr. Welch began his medical career as an orderly at Good Samaritan Hospital in Phoenix, Arizona in 1972.

Brett Willden, DO |
Dr Willden currently works full time running his own family practice office in Anthem, Arizona. He has always had interest in urgent care/emergency medicine. His first job out of residency was working at John C Lincoln Deer Valley ER as well as Mendy's Place. He has also worked for over 8 years in a Family Practice setting where he owned and managed a very successful practice in Anthem. Dr Willden enjoys taking care of families, especially their urgent needs. Dr Willden is board certified in Family Medicine. He is also ACLS, BLS and PALS certified. He graduated from Kirksville College of Osteopathic Medicine where he finished among the top 10 students in his class.

Debbie Sullivan, PA-C |
Debbie Sullivan, who graduated from a Physician Assistant Program in 1990 is board certified as a PA in Primary Care and Surgery, and has maintained certification since her graduation. She has over 18 years of experience in Emergency Medicine and Urgent Care, and has also worked in Family Practice, Pediatiatrics, Orthopedics and Plastic Surgery.
Ms. Sullivan was a founding faculty member at the Midwestern University PA Program in 1997. She worked at Midwestern from 1997 to June 2009. She developed and taught the Emergency Medicine course to PAs, and has assisted in teaching the Emergency Medicine Elective to medical students for the past 3 years. Ms. Sullivan has served on university, state and national boards including the Society of Emergency Medicine Physician Assistants. She served for eight years as a member of the test development committee for the national Physician Assistant Cerfification Examination. She is also a published author and frequent speaker at national conferences.
In addition to her experience as a PA, Ms. Sullivan has also been a Registered Nurse and a certified Emergency Medical Technician. She has worked in small rural hosptials where she was the only Emergency Provider, and has also worked in a large trauma center.
